Due to some of the treatments we perform here at The Boutique Skin Clinic we do require you to fill out an in depth consultation form. This allows us to better understand what we are seeing on your skin. Giving us a full picture makes it easier for us to determine the root cause of why we are seeing what we are seeing and allows us to personalise your treatment based on this. 

There are many things that play a part in the condition of your skin such as medication you are taking and have taken in the past. Health conditions also play a roll in what is happening on the surface of the skin. This all may sound a little intrusive but it allows us to really understand what is presenting to us. 

The information you provide us is extremely confidential. We pride ourselves on this and will NEVER share your information to anyone outside The Boutique Skin Clinic.
